I was always taught that the official baptismal registers, and the records therein, are the property of the Church. The most that could be done in these cases is to add a notation that the baptized person no longer wishes to acknowledge their baptism. This could constitute apostasy by formal act, which incurs excommunication latae sententiae. It does not mean that the person is no longer obliged to follow the precepts of the Church.
